The Manchester City and England winger met Paige Milian as a teenager during his time at QPR.
The Sun reports Sterling asked the 22-year-old to marry him three weeks ago, giving her a huge diamond ring - which she has been flaunting while on holiday with friends.
The couple are reportedly planning to tie the knot next year and are considering a ceremony in Jamaica.
They celebrated the arrival of son Thiago last year.
Sterling has another daughter, Melody Rose, from a previous relationship.
The 23-year-old has enjoyed his best form at Manchester City this term, with Pep Guardiola's side closing in on the Premier League title.
They need to win their next two matches - against Everton and rivals United - to be crowned champions.
Sterling has scored 20 goals in all competitions so far this term.
